We are pleased that you have chosen us as your destination aesthetic spa for cosmetic and spa services. When you book your first appointment, a new patient packet will be emailed to you to fill out prior to your appointment or you can fill out the forms at our office before your appointment. Below are just a few of our standard policies that may be helpful to know prior to your appointment. If you have any questions, please contact us. We look forward to seeing you soon!

Arrivals and Cancellations

On the day of your first appointment, please arrive 15 minutes early to fill out a few forms of information for us, or you may download these new patient forms below. If you need to cancel or reschedule your appointment, please contact us 24 hours in advance so we can reschedule your service for another day. Thank you for your cooperation in being prepared for your appointment. We are looking forward to meeting you and helping you with your skin care needs!

Payment Method

We accept the following forms of payment at the time of service.

  • Visa, MasterCard, American Express, and Discover
  • Debit cards with a Visa or MasterCard logo
  • Cash
  • Personal checks from a local bank

Return Policy

Skin Care By Design accepts the return or exchange of unopened, undamaged products within 14 days of the original purchase. A container is considered to be open if the safety seal is broken and may not be returned or exchanged. We gladly provide samples of most of our products prior to your first purchase. This allows you to ensure that the product characteristics are acceptable and well-tolerated before purchasing the product. Products purchased at a promotional discount are not returnable.


We perform many services that are appropriate for children. However, everyone under the age of 18 receiving services from Skin Care By Design must be accompanied by a parent or legal guardian on each visit and the parent or legal guardian must sign the appropriate Informed Consent forms for each visit. We request that clients receiving services do not bring children under the age of 13 to their appointment unless the children are accompanied by another adult.

Pet Policy

We love animals, but as a medical facility, we do not allow animals in the facility except for certified service animals that are on a leash. Thank you for your cooperation.
